кажется, что-то не так в дастком королевстве... Какая версия modutils?
У меня 2.4.10-alt1 - все нормально, хоть и нет такого девайса.
Попробуйте пока просто
/sbin/insmod /lib/modules/2.4.9-alt4-up/kernel/drivers/scsi/ide-scsi.o
