Рекомендую обновить libqt3 до >= 3.3.2-alt4
Создайте новый файл main.cpp в диалоге нового файла.
Возможно designer придется запускать
LC_ALL=C designer-qt3, т.к. он переводит лишнее на русский в .pro
и создать проект заново.

Я вижу. Только что это такое и как исправить не пойму...
Форма то пустая, поидее работать должна...
Тем более если попробовать сделать простенький пример их QtDesignet
Manual quickstart такая же фигня...
Хотя там все дотошно расписанно, куда ткнуть чтобы открыть новый проект
и т. д.
