Например, из-за неровного отмонтирования.

В качестве паллиатива по данной ситуации (хотя и вообще полезно):
mdadm -G /dev/md0 -b internal
(см. описание write intent bitmap в манпейдже; спасибо mrkooll@
за то, что в своё время подсказал про эту фичу)
