У вас initrd не нашёл root. Проверьте в появившемся initramfs, собрался
ли raid (cat /proc/mdstat), и, если он собрался, повесьте багу на
make-initrd (желательно приложив сразу вывод make-initrd
bug-report). Если raid не собрался, то тоже надо вешать багу о том, что
не собрался и на тот же make-initrd ;)

В том смысле что разбивка делалась fdisk`ом на физ. sda, а для md
сделано mkfs.ext3 /dev/md1?
т.е. все в порядке? или нужно было по другому?

Чтоб спокойней -- стоит ещё обратить внимание на /etc/mdadm.conf
по части MAILADDR (проверить достижимость админа по указанному
адресу), ну и проверить (см. mdadm(8) насчёт TestMessage).
