Проверьте. что в систему не затесался multipath.
Если да, то удалить его или прописать диски в секцию blacklist
/etc/multipath.conf (например, так: wwid ST3500320AS*).
Но лучше удалить multipath вообще, почему-то blacklist, при некоторых
условиях, не срабатывает.
Вообще, эпические грабли (я на них тоже нарвался, потерял данные. И до сих пор
не могу понять, КТО мне поставил multipath! :)
