Просто emacs - это запуск самостоятельным окном? Попробуйте запустить
emacs -nw из xterm или подобного эмулятора терминала (или просто emacs
из консоли). Есть подозрение, что emacsclient здесь не причем. Перед
тем, как зависнуть случаем не "Starting new Ispell process ..." пишет?

Ничего особо сложного тут нет. Файлы расширения ложатся в подкаталог
%_emacslispdir, конфиги для их запуска в %_emacs_sitestart_dir - вот в
принципе и всё.
Почитайте /usr/share/doc/emacs-devel-0.0.2/README.ALT-ru_RU.KOI8-R из
emacs-devel, там описаны макросы из rpm-macros-emacs и некоторые
особенности сборки расширения под альтовский emacs. Потом берете спек из
какого-нибудь не очень сложного расширения и модифицируете под себя.
