Раньше он шел вместе с postgresql; сейчас стал отдельным и живет на
http://www.pgaccess.org. Что, неужели этого до сих пор больше никто не
заметил?

Если только для совместимости. Впрочем, они маленькие.

Возможно (если бы у postgresql-tk висела зависимость на server, это
было бы полным безобразием; сейчас ее нет - при отсутствии сервера
просто ставятся лишние файлы).
postgresql-python, кстати, точно так же свален в кучу.
О, кстати, а в описании пакета postgresql-tk так и остался pgaccess.
