Наверное, какие-то пакеты необходимо установить

Попробовал задать
lpr -P HP4L -o media=Custom.208x296mm
или
lpr -P HP4L -o media=Custom.296x208mm
Странно, но один и тот же результат -- печатает, со смещением верхнего края
вверх, примерно, на 20 мм
Правда правую границу уже подрезает меньше на 1-2мм
