Взаимонепонимание DMA-режима контроллером и устройством. Судя по
приведенному вами логу, скорее всего, конфликт сидирома и винта.
Также вероятно, что используется неподобающий шлейф
(40-жильный).
Нет ли возможности разнести диск и сидиром по шлейфам? И вообще
попробовать убрать все IDE-устройства, кроме hda и посмотреть.
Потом подключать по одному и выявить поганца.

Он вроде бы на 5400 rpm? 20Мб/с для него -- вполне и вполне
нормально, тем более что UDMA66.
