Когда модули не проименованы такое бывает.
Нужно прописывать alias в modprobe.conf.

Лучше
USE_HOTPLUG=no
MODULE=<что надо>
в опциях интерфейсов.

В /etc/iftab был указан два раза с одинаковым mac. Происки альтератора. ;)
