Можно эту команду в /etc/rc.d/rc.local.
Всегда при загрузке будет создаваться.
Можно посмотреть /etc/udev/rules.d/* на предмет modem, и
сделать как там с ttySLM0. (Как точно я не знаю)
И передернуть udev
service udev restart
Только при обновлении udev слетит, нужно будет опять править.
Я так с tun мучаюсь.
