pam_warn.so добавляют не для паранойи а для отладки.

Это нормально.
> account required pam_permit.so
Вообще-то pam_userdb поддерживает account, но ничего по существу в этом
режиме не делает. Так что, пожалуй, можно оставить и pam_permit, хотя
и pam_userdb тоже должно работать.
