Сообщения от монитора типа "out of range" я получал обычно именно
когда выставлял слишком большой refresh. Бывало такое и при
refresh _близком_ к предельному, если менял разрешение. А
монитор, часом, не ЖК? У них refresh, вроде как, фиксированный.
Там игры с частотами опасны. У вас modeline в конфиге как
прописаны? Как рассчитаны?
