Возможно, mod_rewrite раскомментирован позже, чем включен конфиг,
подключающий тот виртхост, где его пытаются задействовать (ошибка
вроде простая, но додуматься до неё бывает долго).
Возможно, не разрешено его использование (кажется, Options).
Надо свериться с документацией mod_rewrite (сам перечитываю чуть
ли не каждый раз) и, возможно, задрать уровень подробности лога.
