А зачем сейчас в initrd dm-mod? lvm/evms там всё равно пока не
работают.

mkinitrd-3.0.5-alt1, где IGNORED_MODNAMES не влияет на включение
модулей, явным образом указанных в командной строке, уже в пути.
Автоматическое определение необходимости usb-storage пока не работает,
но хотя бы --with usb-storage работать будет (можно попробовать и
--extra 'uhci-hcd ohci-hdc ehci-hcd usb-storage', чтобы нужные модули
загружались через udevd при необходимости).
