Что-то я не припомню, что точно я писал, но кажется адрес можно не
указывать т.к. гражданам Украины можно находиться на территории России
без регистрации на протяжении 3 месяцев, а цель поездки можно указать:
частная, личная, отдых, ... Конечно лучше всего проконсультироваться у
проводника, он выдает эти карты. Кстати, проводник может забыть ее
выдать, нужно не забыть проследить за этим. И главное не потерять
вторую половинку карты, ее нужно предъявлять по возвращению.
