Да.
ConsoleOnly и GTK и Gnome и KDE --- это Additional Category.
а в desktop entry должна быть хотя бы одна Main Category.
Т.е. их писать как бы не обязательно.
Но желательно.

Да. для поддержки тех, кто не понимают .desktop напрямую
(для облегчения работы майнтайнеру menu).

Нет, очевидно.

КДЕ очевидно не успевает за ним...
Может, в KDE 4 причешут...
Напомню, спецификации на fd.o постоянно расширяются и допрлняются.
