/etc/init.d/ntpd при запуске ищет адреса для этой подстройки в файле
/etc/ntp/step-tickers. В этом может и крыться ключ к "иногда".

Дело даже не в возможных сбоях DNS. По крайней мере раньше ntpd плохо
относился к доменным именам серверов в директиве restrict, если мне не
изменяет память.
