mdadm показывает не тот UUID, который нужен в данном случае - это UIID
массива, а нужен UUID файловой системы, который выдаёт blkid (либо
/lib/udev/vol_id /dev/...).

Если дело в изменении режима контроллера, это не решит проблему;
необходимо либо вернуть старый режим, либо перегенерировать initrd в
установленной системе.
