Сделать
# control apache-mod_php relaxed
или
# control apache-mod_php public
Поскольку по-умолчанию PHP стоит в restricted, с включенным safe
mode и отключенной phpinfo().
Если не поможет - то стоит убедиться, что этот файл обслуживает именно
Apache 1.3. Проверить, не запущен ли Apache 2.2 вместо Apache 1.3.
Проверить, подгрузился ли mod_php - включить mod_info и посмотреть на
http://127.0.0.1/mod_info
Ну и т.д.
