учите матчасть =)) block-major-8 - это scsi диск, а у вас в ядре и initrd
нет поддержки scsi дисков, т.е про sd_mod вы забыли (привет сборщикам
ядра, экономящим на спичках). Сам недавно на подобные грабли наступил...
Решение проблемы mkinitrd --preload sd_mod --with=<модуль raid
контроллера> и будет счастье
