Файловая система?
RaiserFS?
Может быть у вас нарушения в структуре дерева.
Тогда поможет:
raiserfsck --rebuild-tree /dev/xxxx
Если все же винт:
Попробуйте для начала badblocks /dev/xxxx _БЕЗ_ ключа w!
