Среди Ваших действий нет изменений в master.cf:
#smtp inet n - y - - smtpd
smtp inet n - y - - smtpd-sasl
Не сделали или не не написали?
Еще раньше был баг - saslpasswd добавлял юзера только со второго
раза. Не знаю как сейчас с этим дела.
